OWENSBORO, Ky. — The Green River District Health Department is currently in the process of conducting a community health assessment. The assessment includes a survey collecting information from the community regarding what they feel are the community’s most concerning health needs.
To reach a wider audience and gain more significant insights into the needs of the populations served in our community, GRDHD has made the survey available by clicking here.
The survey will also be shared on the GRDHD website and social media pages, such as Facebook and Twitter. A flyer with a QR code can be found below. A translation feature embedded within the survey allows the participant to complete the survey in English or Spanish.
The link to the survey will be open until Monday, May 8.
The data collected during this process will be discussed and analyzed by community partners to help determine priority health areas to focus on over the next three to five years.
