Melissa Jane Berkley, 18, died Saturday Nov. 17, 2012, at University of Louisville Hospital in Louisville, Ky. She was born in Owensboro to Tony Gene and Melissa Faye Filback Berkley.
Survivors include her parents, Tony Gene and Melissa Faye Berkley; three brothers Jerry Hayse, Jr., Thomas Clay Hayse and Tony Gene Berkley, Jr.; a sister, Rosemary Lucille Hayse; grandparents Vera C. Griffith, Mazena Berkley and Jimmy Griffith and three nephews.
Services are 2 p.m. Wednesday at Bevil Bros. Funeral Home in Beaver Dam. Burial will be in Mt. Pleasant Cemetery near Rosine. Bro. Ray Austin will be officiating. Visitation will be from 4 p.m. until 8 p.m. Tuesday and from 9 a.m. until funeral time on Wednesday.
