Jack Henry Hall, infant son of Andrew and Jazzy Hall, of Fordsville, was born and died on May 19, 2015, at Owensboro Health Regional Hospital.
Also surviving are paternal grandparents, Darren and Tammy Hall, of Fordsville, and maternal grandparents, Chris and Julie Duncan, of Fordsville.
Graveside service will be at 2 p.m. Thursday, May 21, at Beatty Cemetery. Geary Funeral Home, Fordsville, is handling the arrangements.
